What is IUI, and how does it work?

IUI stands for Intrauterine Insemination. It is a fertility treatment where prepared sperm is placed directly into the uterus around the time of ovulation.

The idea behind IUI insemination is to shorten the distance sperm needs to travel, giving it a better chance of reaching the egg. Unlike IVF, fertilisation happens inside the body rather than in the laboratory.    This means IUI offers a less invasive approach, which may be well-suited to some patients depending on their fertility history and treatment goals. 

An IUI cycle still involves medication, monitoring, scans and testing. It can be used with partner sperm or donor sperm, depending on your treatment plan. Because no embryos are created during IUI, each pregnancy attempt requires a separate cycle.

The IUI process

    Ovarian stimulation & monitoring

    The first stage of the IUI procedure is ovarian stimulation. Oral or injectable fertility medication is used to encourage the ovaries to develop one or two mature follicles. Treatment usually lasts around 12 days, depending on your protocol and how your body responds.

    You will attend monitoring appointments so we can track the number and size of developing follicles using ultrasound scans. The aim is usually to support the development of one or two eggs. When the follicle(s) are ready, a trigger injection is given, and insemination is planned for the following day.

    Sperm preparation

    Once a mature follicle is confirmed, the trigger injection helps the body release the egg at the right time for treatment. Timing is one of the most important parts of IUI, which is why cycle monitoring is such a key part of the process. If you are using partner sperm, a semen sample is usually provided on the day and then prepared in the laboratory. If donor sperm is being used, the frozen sample is thawed and prepared for treatment. In both cases, the aim is to concentrate the healthiest motile sperm into a small volume ready for insemination.

    Insemination

    The insemination itself is quick and straightforward. A fine catheter is passed through the cervix so the prepared sperm can be placed directly into the uterus. For most people, it feels similar to a smear test and is only mildly uncomfortable. You do not need sedation, and the procedure usually takes 10 minutes or less. Afterwards, you can return home and continue your day as normal. After your IUI, you will attend the clinic about 12-14 days later for a blood test to check if you are pregnant.

Who is IUI suitable for?

IUI may be suitable for patients whose fallopian tubes are functioning and where sperm quality is sufficient for treatment. It is often considered for people who have been trying to conceive without success, and it can also be an important option when donor sperm is being used. 

Your age, fertility history, hormone levels and any underlying conditions all help guide whether IUI is a realistic option. If IUI is unlikely to offer a good chance of success, we will explain that clearly and talk through alternatives.
